Patient Centered Medical Home staff position responsible for assisting patients in finding new ways to inspire healthier habits and attain patient established health related goals. The individual will work with patients: under the direction of the PCMH Lead and practice clinicians; working collaboratively with other members of the health team, using motivational interviewing techniques, behavior modification techniques to set goals and establish action plans to promote wellness and help individuals realize their part in maintaining their health. RN currently licensed in the state of Vermont,Registered Dietitian withactive status on the Commission of Dietetic Registration, orSocial Worker Masters prepared with Vermont licensed or ability to obtain license. Excellent interpersonal, communication and writing skills are essential with the ability to relate and motivate effectively with a diverse group of patients and clinical staff. Demonstrates computer competency including all Microsoft Office applications.
3 years of health care experience in field including experience with assessment, motivational interviewing techniques, healthy living, therapeutic lifestyle change and counseling. Electronic medical record experience preferred.
